Home and Community Care    

Home and Community Care program provides a comprehensive, coordinated and integrated range of basic maintenance and support services for frail aged people, people with a disability and their carer's.  The program supports these people to remain independent at home and in the community, thereby enhancing quality of life and preventing inappropriate admission to long term residential care.  

              

Eligibility for HACC Services       

You may be eligible for HACC Services if you are: 

  • A frail aged person

  • A person with a moderate, sever or profound disability
  • The unpaid Carer of an eligible client
  • Eligibility to HACC services is not automatic, and services are allocated to those with the greatest need.
  • You do not have to provide evidence of residency in order to receive a service.
  • The waiting time can vary for different services
  • HACC services are provided as a low cost service
  • No person is denied help if they cannot afford to pay. Any service costs will be discussed with the service provider.

          

Community Care Access Point     

The Community Care Access Point (CCAP) is a single contact point that provides a simple way for people to get information about and gain access to community care services in the Hunter and Central Coast Regions.

Councils Role in HACC      

Gosford City Council employees staff in a Development and Support role to HACC Service Providers. We do not oversee the programs, or provide direct client services. Council auspices the Central Coast HACC Development Project and the Central Coast/Hunter HACC Training Project.
