Gosford City Council Home and Community Care Program
Home and Community CareHome and Community Care program provides a comprehensive, coordinated and integrated range of basic maintenance and support services for frail aged people, people with a disability and their carer's. The program supports these people to remain independent at home and in the community, thereby enhancing quality of life and preventing inappropriate admission to long term residential care.
Eligibility for HACC ServicesYou may be eligible for HACC Services if you are:
Community Care Access PointThe Community Care Access Point (CCAP) is a single contact point that provides a simple way for people to get information about and gain access to community care services in the Hunter and Central Coast Regions. If you would like to seek services of the HACC program, please contact CCAP 1300 731 556
Councils Role in HACCGosford City Council employees staff in a Development and Support role to HACC Service Providers. We do not oversee the programs, or provide direct client services. Council auspices the Central Coast HACC Development Project and the Central Coast/Hunter HACC Training Project.
